Robert Baker-Hargrove oversees clinical and administrative operations at Our Family Health, bringing more than two decades of executive leadership across healthcare and nonprofit organizations.

He was previously Co-Founder, COO, and Co-CEO of 26Health, Central Florida's leading healthcare provider for LGBTQ+ and allied communities, where he helped scale operations, expand services, and build culturally competent care models. Before moving into healthcare, he spent more than twenty years at The Walt Disney Company in entertainment operations and production management — experience in complex, large-scale operations that shapes how he builds teams today.

He has served on the board of the Mental Health Association of Central Florida, including a term as board chair, and on the Florida State Board of Directors for Mental Health. He holds a Bachelor of Science from the University of Texas at Austin and an MBA from Webster University.

Dr. Hayat Heriba serves as Medical Director of Our Family Health, providing clinical leadership and oversight across the health center's medical services. A board-certified family medicine physician with more than 35 years of practice in Central Florida, she brings deep experience in preventive care, chronic disease management, and care models built around the realities patients actually face.

As Medical Director, she oversees clinical quality, patient safety, and provider performance, and leads the development of care protocols and quality improvement work across the organization. She supervises care teams, strengthens coordination with specialty and hospital partners, and works to expand the center's clinical capacity as it grows.

She has served on the Immunization Collaboration Committee and the National Clinical Research Network of the American Academy of Family Physicians, and on the Pharmacy and IT Committee at AdventHealth.

Her leadership is grounded in a long-standing commitment to health equity — expanding access for individuals and families who face financial, linguistic, and cultural barriers to care, and building a clinical environment where every patient is treated with dignity.

Susan Phelps leads behavioral health at Our Family Health, bringing more than 25 years of experience as a clinician and a builder of behavioral health programs across outpatient, residential, and community health settings.

Before joining Our Family Health, she served as Director of Behavioral Health and Collaborative Care Management at 26Health in Orlando, where she built integrated care programs, developed clinical policy, and led quality improvement work under federal health center standards. Earlier in her career she served as Clinical Director of a residential program and led admissions and utilization management for an adolescent treatment center.

As Behavioral Health Director, Susan is responsible for the clinical quality and growth of the behavioral health team — creating an environment where clinicians are equipped for and genuinely invested in the work. She believes healthcare is a right rather than a privilege, and that the best outcomes come when a patient's providers work as one team rather than in isolation.

Omar Heriba manages operations at Our Family Health, focused on the systems and processes that let providers spend their time on patient care rather than paperwork.

He brings more than 16 years of experience across healthcare, academia, and the federal government, with expertise spanning neuroscience, ophthalmology, primary care operations, patient relations, medical records, compliance, and billing. He has held positions with the University of Central Florida's Burnett School of Biomedical Sciences, private medical practices, and the Federal Geographic Data Committee within the U.S. Department of the Interior, on projects ranging from local initiatives to global applications. Throughout, he has worked closely with physicians, nurses, and care teams to support the kind of operational readiness that lets a practice actually deliver on what it promises patients.

Omar holds a Master of Public Health in Health Equity from Boston University, along with degrees in biomedical sciences and computer science from the University of Central Florida and the University of Florida.
