Getting Started
Our Family Health (OFH) is a community-focused primary care practice dedicated to helping individuals and families live healthier lives. We provide compassionate, evidence-based care in a welcoming environment where every patient is treated with dignity and respect.
We welcome adults, families, insured, uninsured, and underinsured patients. Our goal is to remove barriers to care so everyone has access to quality healthcare.
Call our office or request an appointment online. Our staff will guide you through registration, verify insurance if applicable, and schedule you with the provider who best meets your needs.
Bring a photo ID, insurance card (if applicable), medication list, medical records you would like your provider to review, and any questions you have about your health.
Appointments
Appointments may be scheduled by phone or through our patient portal when available.
Some same-day appointments may be available. Please call first so we can direct you to the appropriate level of care.
Please notify us as soon as possible so we can offer that appointment to another patient.
Visit length depends on your healthcare needs. New patient visits generally take longer than follow-up appointments.
Primary Care
We provide preventive care, annual wellness visits, management of acute illnesses, chronic disease management, medication management, laboratory coordination, referrals to specialists, telehealth, HIV prevention services, and care coordination.
Yes. We help patients manage conditions such as diabetes, high blood pressure, high cholesterol, asthma, and other ongoing medical concerns.
Absolutely. Preventive care, screenings, immunizations, and healthy lifestyle counseling are central to our approach.
Insurance & Payments
We accept many Medicare, Medicaid, Marketplace, and commercial insurance plans. Contact our office to verify your coverage.
You can still receive care. We offer self-pay options and may be able to help determine eligibility for financial assistance.
Eligible patients may receive discounted fees based on household income and family size. Our staff can help you complete the application.
Telehealth
Yes. Many follow-up and routine appointments can be completed securely through Healow telehealth.
A smartphone, tablet, or computer with internet access, a private location, and a few minutes before your appointment to log in.
No. Some conditions require an in-person examination or testing. Your provider will recommend the best type of visit.
Patient Portal
The portal allows you to request appointments, review portions of your medical record, request prescription refills, exchange secure messages with your care team, complete forms, and access telehealth visits.
Yes. We use secure technology that complies with HIPAA privacy and security standards.
Yes. Healow is available as a free app for Apple and Android devices.

Yes. Confidential HIV testing is available in a supportive, stigma-free environment.
PrEP is a medication that greatly reduces the risk of acquiring HIV when taken as prescribed. Our providers will determine whether it is appropriate for you and provide ongoing monitoring.
